Making Appointments
We are available to see patients by appointment at our two office locations. The time allotted for each appointment depends on the nature of your problem. We ask that you give our receptionist enough information to assign priority and time to meet your needs. Please call several months in advance when scheduling routine exams, so that an appointment can be made that will be the most convenient for you.

Please do not e-mail us to schedule, reschedule or cancel an appointment. There are many variables in scheduling an appointment and it is best done by telephone with one of our experienced staff.
Please call as soon as possible if you cannot keep your scheduled appointment.
On your first visit, we will begin a permanent record for you, and you will be asked to fill out a patient information sheet. (For your convenience, you can print our Patient Information Sheet by clicking on the link on our front page.) Please bring your insurance information with you to help complete this form.
A personal and family health history will be taken before your examination and consultation. If you have any questions regarding your visit, do not hesitate to ask.
Phone calls for appointments can be made between 8 a.m. and 5 p.m. Monday through Thursday, and between 8 a.m. and 4 p.m. on Friday. Appointments cannot be scheduled, rescheduled or cancelled via e-mail.
