Fees and Insurance

We will file your insurance promptly.  Contemporary Care for Women is contracted with most insurance plans, including the plans listed at the bottom of this page.

Most managed care insurance plans require a co-payment at the time of service. The out-of-pocket expense for your visit depends on the contractual agreement with your insurance plan. Insurance co-pay, deductibles, or co-insurance may be due at the time of service; however, by staying within your managed care network, you can usually avoid extra costs.

If you do not have insurance, payment is expected at the time of service. If you have financial problems that delay the payment of your bill, the Billing Office will make every effort to work out a payment plan to enable you to take care of your obligation.

If you’re confused about all of this, just talk to us!  We deal with insurance all the time and will be happy to explain it to you in a way that you understand. Call our Billing Office at 480-505-4258 for help or with questions about your billing statements, filing insurance claims, payment arrangements, or fee policies. Phone calls can be made between 8 a.m. and 5 p.m. Monday through Thursday, and between 8 a.m. and 4 p.m. on Friday.

For your convenience, we accept MasterCard or Visa.

Please Note:  We are not contracted with Medicare.  If a Medicare patient chooses to see one of our providers, we are happy to do see them on a cash payment basis.  We will not be able to bill any supplementary or secondary insurance.  The patient should realize that they may not be able to file either with Medicare, their supplementary or their secondary insurance if they pay cash for their visit with us as we are not a contracted provider.
